45-49 Health check

As we get older, many of us become more vulnerable to illness. The federal government has recognised this and has introduced a new health check for everyone aged between 45 and 49 who may be at risk of developing a health complaint, like diabetes or heart problems.

The aim of the health check is to help find, prevent or lessen the effect of disease - it is better to avoid disease than to treat it. This health check will give us the opportunity to look at your lifestyle and medical/family history to find out if you are at risk.

The health check would involve:

  • updating your medical history and looking at your health issues

  • doing tests (such as blood pressure tests), if required

  • follow up of any problems identified

  • advice and information, for example on how to make lifestyle changes to improve your health

If you would like a Health Assessment please call and discuss with our receptionists.  An appointment will be booked for 1 hour with our Practice Nurse and a consultation with your regular doctor. 

75 Health check

The aim of the health check is to help find, prevent or lessen the effect of disease and identify any areas where assistance may be required to improve your lifestyle. The health check would involve:

  • updating your medical history and looking at your health issues

  • doing tests (such as blood pressure tests), if required

  • follow up of any problems identified

  • advice and information, for example on how to make lifestyle changes to improve your health.

Chronic disease management

If you suffer from a chronic illness  your illness may be better managed with a comprehensive, structured GP management plan.

Your GP management plan will be specifically tailored to your health needs and will:

  • Set out the services to be provided by your GP

  • List the actions you can take to help manage your condition

  • Set goals for your treatment

  • Be regularly reviewed, ensuring the plan changes along with your needs

As part of your plan referrals can be made to allied health professionals eg. dieticians, physiotherapists, podiatrists etc.  With these referrals in place Medicare will give rebates for the services.

Chronic Disease Management requires a 1 hour appointment with our Practice Nurse and an appointment with your regular doctor.
